With intestinal atresia, atresia refers to a passageway that's closed or gone completely,

so intestinal atresia is when a malformation during fetal development results an absent

portion of the small or large intestine.

If instead the passageway was just narrowed, then it's referred to as intestinal stenosis—oftentimes

these are both just lumped together as intestinal atresia and stenosis.

The specific type of intestinal atresia is named depending on what section of the intestines

is affected.

Duodenal atresia or stenosis is where the first section—the duodenum—is affected.

The underlying cause of duodenal atresia isn't known, although it's commonly associated

with trisomy 21, also known as Down syndrome, suggesting that it's likely due to genetic

defects affecting normal development.

Although it isn't known why intestinal atresia develops isn't known, it is fairly well

known how it develops.

Normally, early on in development, the gut's a hollow tube.

But by around 6 weeks gestation, the epithelium of the duodenum proliferates and it ends up

plugging up the lumen making it a completely solid stick of tissue.

After that, the cells in the middle undergo apoptosis, or programmed cell death, and by

9 weeks gestation, the tube's hollow again, called recanalization.

This entire process is called vacuolation.

In duodenal atresia, this vacuolation process fails, and the duodenum doesn't recanalize

properly, resulting in atresia or stenosis of the duodenum.

Non-duodenal intestinal atresias or stenoses, like those affecting the jejunum or ileum,

or even affected the large intestine like the colon, are generally not a result of recanalization,

and instead are more likely to result from ischemic injury, meaning lack of bloodflow,

to the developing gut.

Now, the jejunum, ileum, and large intestine rely on blood supply from the superior mesenteric

artery, while only part of the duodenum does.

So for example, looking at the jejunum, if the vessels supplying it with blood are occluded

then decreased perfusion leads to ischemic necrosis, which leads to segmental reabsorption

and narrowing or complete destruction of the affected area.

This classically leads to an "apple peel" configuration due to it's spiral shaped.

Now, there are also some important clinical signs of duodenal atresia.

First, usually the developing fetus swallows and digests some amniotic fluid, but since

a segment of the duodenum is narrowed or absent, the fluid has nowhere to go.

This leads to distention in both the stomach and duodenum, Which causes a "double-bubble"

appearance on x-ray as a result of the inflated stomach and duodenum being separated by the

muscular pyloric valve.

This distention can also be detected on a prenatal ultrasound.

Also, though, since the fetus swallows less amniotic fluid, that means more fluid sticks

around in the amniotic sac, which is called polyhydramnios.

Finally, the newborn might have bilious vomiting in the first few days of life, since bile's

secreted into the duodenum can't be excreted and gets regurgitated back up with breast

milk or formula, which also has no place to go.

Treatment of intestinal atresia involves gastric decompression, which is removing fluids from

the stomach with a nasogastric tube, as well as making sure the newborn receives fluids

intravenously.

The definitive treatment of intestinal atresia, though, is surgical removal and repair of

the affected segment, which in the case of duodenal atresia is called a duodenoduodenostomy,

since you're attaching duodenum to duodenum.

Typically, prognosis following surgery is very good.

Alright, as a quick recap, intestinal atresia and stenosis is where some portion of the

intestine—which could be the small intestine or large intestine—is absent or narrowed.

Duodenal atresia is typically a result of a failure of recanalization, whereas non-duodenal

atresias and stenoses are thought to be caused by ischemic injury.

SPECIAL VIDEO #4: TOP 5 BEST AND WORST GAMES OF 2016 - Duration: 6:35.

HAPPY NEW YEAR, MY FELLOW EXPLORERS!

Now that I'm done with the movies, it's time for the videogames, and yes, there's

only five for the tops and bottoms this time because I was only able to play so many games;

my PC is starting to give up on the latest triple A titles and I don't have any next-gen

consoles, so I had to rely on friends for some of the games I'm about to mention on

this list.

As usual, allow me to make some very powerful honorable mentions on what I consider to be

the best games released this year --- or at least any available ports released this year,

and on that note, I have to begin with:

DANGANRONPA 2: GOODBYE DESPAIR

I don't give a shit if the original game is four years old; I was unable to play it

until now, because I don't know Japanese and I'm NOT paying for a PS Vita.

That being said, I'm really glad they finally ported it to PC, because I can finally experience

the second act of the Danganronpa franchise.

Not only that, but in 60 spectacular frames per second.

And boy, was it good.

Also, DRAGON BALL FUSIONS was VERY good; it was essentially a portable Dragon Ball Xenoverse

with an extreme focus on fusions for every character, and boy oh boy, did they deliver

on that front.

There are about 5 thousand different combinations for any two Dragon Ball characters and it

covers the original Dragon Ball, Z, GT and most of the Super saga so far.

I'm sure it will annoy some that I didn't put INSIDE on the list, and while I do have

a review for it coming soon SPOILERS, I guess the least I can say to justify why it's

on my honorable mentions is that it's the prettiest, darkest, most shockingly confusing

game where all you do is run to the right until something forces you to stop.

And finally, PONY ISLAND.

You probably didn't hear about this one, and you should, because it was a very interesting

game that involves a game made by Satan with some of the most unique game mechanics I've

seen so far.

Alright, honorable mentions are over, so let's start from the top and work our way down.

Just the way I like it.

Number 5 is yet another indie game that most people probably won't recognize, unless

you've been in several game awards where this game was showcased, and it's no surprise,

because HUE is just simply amazing.

The inventive color-changing technique that interacts with the environment, the Limbo-esque

design choices and the overall feel of HUE have garnered a plethora of awards to its

name and a safe spot on my list of the best games of the year.

Similarly, the fifth worst game is something no one will recognize unless they're looking

for cheap Donkey Kong Country Returns meets Yoshi's island knockoffs,

so SHU is no surprise on this list.

Uninteresting, unforgiving and unimpressive, this is an adventure that's better off cast

into the wind.

Ever since I discovered the Danganronpa franchise, I've been having an affair with their developers,

but we all know that I'm married to Pendulo Studios, the Spanish masters of the graphic

adventure genre,

so when they released YESTERDAY ORIGINS I was all over it like a hound on fresh meat,

and BY GOD was it good.

They improved a lot of the visual elements and the gameplay mechanics, the prequel-sequel

setup was well thought out and implemented correctly, and there were a lot of small details

and easter eggs that made Yesterday: Origins one of the best graphic adventures of recent years.

Good job, Pendulo Studios.

Keep doing your thing and stay away from nasty corporations, and you'll be fine.

It pains me to put this game on this list because it's a crossover (if you want to

put it that way) of two of Nintendo's most popular RPG's, but what do you want me to

tell you?

MARIO AND LUIGI: PAPER JAM is proof positive that you can simultaneously destroy two of

your most popular spin-off franchises if you start making Toad rescue missions mandatory

and send Paper Luigi to the Sound Test screen.

Well, this is as late an entry as late gets, but that's the problem with most products

that come out of Kickstarter; delays are mandatory, and the more, the merrier, according to some.

However, for the longest time I could have sworn this game was made by Ubisoft,

because SHANTAE: HALF GENIE HERO looks exactly like a game made with Ubi Art Framework.

This is perhaps the cutest game I think I've ever played, and it's all on the art style.

Sure, the transformations are nice, the story is wacky and pretty self-aware, the level

design is gorgeous, but it's all about Shantae crawling.

It's so damn cute.

Speaking of kickstarted projects and delays, this game wears them like a badge of honor

for taking not one, not two, but almost three years in development, and that would be no

biggie, if it wasn't a crowdfunded project.

Obviously, I'm talking about the star of the very first video I made for this channel.

"Disappointing" doesn't begin to cover what most people felt when this game finally

came out after what looked like an eternity because it was the epitome of discordance

between gameplay and level design, a lot of broken promises and a waste of 4 million dollars.

Well, you know what they say, most people make life-changing choices when they're

at their 20's,

and POKÉMON SUN AND MOON are no different.

You could tell that those games were a very risky play for Gamefreak with the alolan forms,

no gyms and some of their new Pokémon designs, but you can't argue with the end result

that maximized the presentation, polished the core gameplay for newcomers and shook

up the metagame for experienced players.

Risk big, win big, they say.

Whaddya know, I'm a cheater.

I'm repeating an offender here, and it's none other than

PAPER MARIO: COLOR SPLASH.

You couldn't just leave it at Paper Jam, could you.

I mean…

What happened, Intelligent Systems?!

You used to make really good Paper Mario games!

What made you think Sticker Star was good?!

What made you think that this kiddy-kiddy, Toad-emphasis on your Role-playing games was

the right direction?!

And no, you can't come and say it's because it's a Nintendo system, because The Thousand

Year Door and Super Paper Mario were far edgier than this and they were so much better!

You know what, go back to making Fire Emblem.

At least those games are still good.

You know a game is good if it makes 3 grown men playing it cry like babies, and it's

not so much on the ending but the fact that it was finally released and it was as good

as everyone expected it to be, and I think it's mostly because of the open world, charming

characters and wonderful environments; stand up in applause for the boyband;

it's FINAL FANTASY XV.

I'll get into detail on my review for it, but this game was as epic, as big, as charming,

as mysterious, as beautiful and as entertaining as Final Fantasy 10, and that's saying a

lot for a game that spent 10 years in development.

On the other hand, the hype for number one worst didn't work on me, and being on god-knows-how-long

in development didn't exactly do it justice either.

NO MAN'S SKY is pretty much the negative of Final Fantasy XV, though they're more

similar than I'd like to admit: this game *was* big, epic and open-world…

A big fat lie, an epic mess, and an open world --- or should I say, an open universe --- of

uninteresting sequences of events that ended with the center of the galaxy, a game reset

and my fist across my monitor.

Well, those are the best and worst games of 2016 in my opinion.

There were a lot of surprises on both ends, both in quality of triple A titles and on

the indie scene.

I can't wait for 2017 because there are A LOT of games to play and review, so here's

to a wonderful year for the channel and I hope to see you guys very soon!

first recording of 2017! not my first

upload, but my first recording. i have a

lot of great things planned for this

talking about Art Portfolio 2: A Gallery

of Portraits, by Schanes and Schanes. part

2 and a four-part video series about the

different Art Portfolios that feature

when these wonderful artwork!

Art Portfolio 2, or A Gallery of Portraits,

was produced by Schanes and Schanes in

1980, and features eight beautiful plates

that measure nine by 12 inches, that

Wendy originally illustrated in

beautiful full-color pastels. limited to

only 2,000 autographed copies, this

portfolio was made in response to fans

of ElfQuest and Wendy's artwork making

frequent requests for portraits of their

favorite characters. and now for one of

my favorite parts of the videos, we get

to dig-in! so, as we open up we have this

nice opening page here, and that's where

you'll find your number for your

portfolio. mine is number 817 out of 2,000,

and also now--down here they have a nice

description of each one of the

characters that is featured in the

portfolio! so it's kind of cool! it's

a nice little entry for people that aren't

necessarily too familiar with ElfQuest,

but still think they're beautiful! 'cause i mean,

come on, look at these, guys!

alright, let's look at these guys!

so according to the descriptions here, it

says, "Plate 1 is Skywise, impish

stargazer, keeper of the Lodestone, too curious

for his own good!

it says, "Plate 2 is Leetah the Healer, able

to cure all hurts of body and mind with a

I like that description of her!

currently this portfolio is fairly rare

and seems to be going for around sixty

dollars on eBay, or eighty dollars on

Amazon, if you can even find it!

alright, normally this would be my Question and

well, as i said to him, uh-- i kind of like to

go with the flow with my videos. i kind

of seem to-- errr i kind of tend to like to

go with what feels what should be next.

either by what I see, uh-- that's kind of

"trending" on the ElfQuest groups on

Facebook, or what I see a lot of

questions being asked for, or it could

even be that i recently completed a set

of something and I plan to do a video on that.

such as the DC archives set! i just

recently finished it, and i plan on doing

a video here pretty soon about that, but

I know it's not necessarily in

publication order. so yeah, I just tend to

go with the flow i go with whatever

feels right at the time. um, if you have any

I've heard it's dangerous to go to sleep after suffering from a concussion, is that true?

Generally speaking, this is a myth.

However, the initial symptoms of a concussion can be similar to those of a more...

Serious injury to the head,

Which are medical emergencies that warrant immediate attention.

So what steps should I take if I think I have a concussion?

Get evaluated by a physician or healthcare provider trained in emergency management and

concussion evaluation as soon as possible.

Okay, so I have a concussion.

Now what?

It's time for physical and cognitive rest.

When you get a concussion, physical damage to the brain is minimal, but it's still put

in a state where it's unable to carry out its normal functions.

During this energy crisis, any physical or cognitive activity just increases the demand

for energy at a time when there's already not enough.

People should say you should try to think about nothing to let your brain rest,

but how is that even possible?

Just take it easy, try to only do activities that are absolutely necessary,

like brushing your teeth or cooking meals.

What about schoolwork?

Unfortunately, it may be hard to perform cognitive tasks even after other symptoms, like headaches

or dizziness have gone away.

How long will it take till I'm fully recovered?

Symptoms usually last about seven to ten days after you suffer the injury.

However, no two concussion are ever the same and certain things can contribute to

prolonged symptoms. These include:

That's why you should absolutely be evaluated

by a physician as soon as possible following your injury.

What do I do when I start to feel better?

When you're recovering, it's important to follow a gradual return to function.

That means incrementally increasing the activities you're doing in a way that doesn't make any

of your symptoms worse.

Your physician can help you figure out how best to do this.
