so joining us now South Carolina congressman trey Gowdy chairman of the
House Oversight and Government Reform Committee also a member of the House
Intel committee and you were one of those who saw some of the background
documents related to this issue of whether or not there was a spy or an
informant in the campaign late last week anything you can share on a broad scale
about what went down there I think there two things important to understand
number one the source of president Trump's frustration a Brennan said he
should be in the dustbin of history Comey said impeachments too good of a
remedy clapper doesn't like him Loretta Lynch said call it a matter not an
investigation shift said he had evidence of collusion before we even began the
investigation and 60 Democrats have voted to impeach him before Bob Muller
has come up with a single solitary finding that's what's got him frustrated
what should have him hearten is the fact that Chris Rae rod Rhodes and Stein and
all the senior folks at DOJ now were all Trump appointees so here's what's fair
to ask what did the FBI do when did they do it what was the factual predicate
upon which they took whatever actions they took and against whom were they
directed but remember Martha it was President Trump himself who said number
one I didn't collude with Russia but if anyone connected with my campaign did I
want the FBI to find that out it looks to me like the FBI was doing what
President Trump said I want you to do find it out he's not the target
so when Schiff and others don't make that clear they're doing a disservice to
our fellow citizens he's not the target but this raises the question that the
president raised in the in this one of those tweets there were a lot of them
and which we talked about quite a bit here last week is if that were the case
why didn't they give him a little briefing so here's what we found out we
know we do have somebody who asked some questions of George papadopolis we do
have somebody who's asked questions of a Carter page here's what you need to know
I think defensive briefings are done a lot and why the Comey FBI didn't do it I
don't know but Chris ray and rod Rhodes had signed up at least made it clear to
us Donald Trump was never the target of the investigation he's not the current
target in the investigation now keep that can all change depending on what a
witness says but as of now I think Chris Rhea and rod Rosenstein are stunned
whenever people think Trump is the target of their investigation I'll leave
it up to them how to brief the president had a briefing you're talking about
right now was that strengthened when you went into this briefing last week yes I
am I am even more convinced that the FBI did exactly what my fellow citizens
would want them to do when they got the information they got and that it has
nothing to do with Donald Trump all right so given the things that we're
over here on your right hand all the frustrations do you think it's
problematic the way the president has is tweeting about this all the time because
because he feels like he needs to get used to vent he's got to get his message
out there is it legally problematic in your mind what he's doing I think
anytime you create prior statements you give Muller or other folks a chance to
question you on them and ask what was your factual basis why did you say that
the president should have access to the best legal minds in the country and I
think you should take advantage of those and he's got some really good
communicators that are on his staff and at his at his call if I were his lawyer
and I never will be I would tell him to rely on his lawyers
in his comms phone here's one of them Rudy Giuliani's speaking with Bill
Hemmer over the holiday weekend watch this what's wrong with the government
trying to figure out what Russia was up to nothing wrong with the government
doing that everything wrong with the government spying on a candidate of the
opposition party that's a Watergate a spy gate I mean and without any warning
to him and now to compound that to make it into a criminal investigation bill
that's why this is a rigged investigation there are two things wrong
with what the former US Attorney said number one no one knows whether this is
a criminal investigation Mahler was told to do a counterintelligence
investigation into what Russia did a number two President Trump himself in
the Comey memo said if anyone connected with my campaign was working with Russia
I want you to investigate it and it sounds to me like that is exactly what
the FBI did I think when the president finds out what happened he's
going to be not just fine he's going to be glad that we have an FBI that took
seriously what they heard he was never the target Russia sounds to me as if you
would advise him that there's no problem with him sitting down with Robert Muller
oh absolutely no IIIi have always said I think you ought to sit down with Bob Moe
you've told us publicly there was no collusion you've told us publicly there
was no obstruction say in private what you've said publicly limit the scope to
exactly what the what the Muller memo is but but if he were my client and I'd say
if you've done nothing wrong then you need to sit down and tell
Muller what you know you know we had there was one judge who said that the
the scope was all over the place do you feel comfortable with the scope of this
investigation and do you feel like your committee has has been shared with to
the extent that that exists that the scope exists I'm not sure what the scope
of the Muller probe is but I know this Rosen Stein is the one who created a
memo it's not Bob Mulder's fault remember I have I have I've seen the
memo that you've seen also the other memo some of my colleagues want to see
is a more narrow investigate Russia and all anything related to it and as a
throwaway line at the end and of course if there's any criminality look at that
to me we run towards the criminality but I would think everyone would want to
know what Russia did so so let me with whom if anyone is the second part the
first question is what did Russia do let's see try thank you very much good
to see you congressman okay so trey Gowdy is one of the people
who has seen the documents he's read them he's also been briefed in the
classified briefing and is saying that he doesn't have a problem with what the
FBI did as far as an informant in or in or around the Trump campaign here's the
president last night how do you like the fact they had people infiltrating our
campaign can you imagine it was President Trump
himself who said number one I didn't collude with Russia but if anyone
connected with my campaign did I want the FBI to find that out what worries me
beyond the Russians themselves or the collusion issue but the president's
willingness to kind of make broad-based ad hominem attacks against the whole
integrity of the FBI so what about this and where we stand and the Attorney
General Sessions comments all of it let's bring in our panel byron york

down the fort she is okay your thoughts on trey Gowdy and what this means for
the investigation well I think we have to pay very close attention to what Trey
Gowdy's saying because he has extraordinary access to actual
information about this we all chatter about it and speculate all day and all
night but he knows more than 99.99% of us do because he's seen actual documents
so when he comes out and says this was an appropriately predicated
investigation and the informant within the Trump campaign was not a spy were
acting inappropriately in any way I think that matters and many in the media
are seizing on that element of his interview with Martha McCallum but the
quote also to me that sticks out he said quote it has nothing to do with Donald
Trump himself that is significant if Gowdy is saying if there was this
informant which there was what he was examining was not targeting the
president that matters biron that's true I mean as that you
look at the coverage around the horn of the second part you really got chopped
off all day today first part got ignored the the second part I think there are a
couple of reasons to explain this one trey Gowdy as a federal prosecutor I
think has more trust in the Justice Department than Devin Nunez does I think
it's pretty clear there but I knew we should point out that we haven't heard
from Devin Nunez he was out and about and talking a lot until that briefing
and he has not talked about the briefing there are other people who are in the
briefing who have not talked about it as well he is not the
only one but I think the important thing to remember is the house was trying to
get documents about this informant they were trying to say for example did the
informant issue give reports to the Justice Department if the important if
that the informant talked to George papadopolis did you write it up did he
give it to the Justice Department the fact is they haven't seen that stuff yet
so the Justice Department can say we followed all the rules everything's
great don't worry about it we did it by the book but the house hasn't seen the
stuff yet the other question is if the president-elect is getting classified
briefings as president-elect why is he not in the loop if it had nothing to do
with President Trump why is he not told what the informant is trying to do about
finding any Russian connection in those briefings one would think that he had
briefings along the way he was briefed the Clinton campaign was briefed they
were told that this was happening and that this look the people are
questioning Trey Gowdy's are there imputing his integrity his his motive
here today no no no no but I'm talking about his critics here I'm talking about
people calling him the phony saying he's covering up for a corrupt FBI because in
the fever swamps and in tribal battle people want to paint this like he's a
secret Loretta Lynch Hillary Clinton Obama partisan fighting to cover up a
corrupt FBI he is doing this because he knows that what President Trump is doing
is what Senator Warner describes he's launching an assault on the intelligence
community and the law enforcement agencies that we depend on in this
country and he's doing it knowingly what is Rudy Giuliani running around telling
reporters in town the president thinks this is working he's branded at Spygate
it's working it this is a campaign to discredit Bob Muller's findings and
nothing else Trump knows there was no spy
infiltrating his campaign he knows that it wasn't him that they were after that
they were checking out Russians that were trying to get into his campaign and
connect with his campaign but he's doing it anyway okay here you mentioned
senator Warner here's something else under Warner said about the
investigation overall people I've got exhaustion from the day-to-day
back-and-forth of this story I think the American public will be will be tired of
it if this is not wound down in this calendar year I mean I think that's
significant he's part of the Intelligence Committee that was looking
into this who was really aggressive at the beginning and seems to be kind of
saying we got to wrap this thing up by the end of 2018 whether it's before the
midterms or we're after basically echoing the vice-president
from a few weeks ago urging robert muller to get this thing finished up
soon and i think it makes sense on some level investigations don't need to be
unending and forever which seems to be the standard that some Trump critics
want in this case it's a cloud that continues to hang over him if the point
is to prevent the Russians and other hostile powers from influencing our
elections again in the future as they attempted to do in 2016 it would be nice
to know what happened before the next election
so what Warner said there I think you're right is significant a Democrat saying
okay let's go the other story obviously the New York Times had this story not a
surprise to some people that the president may have said why don't you
get back in the game to attorney General Sessions on his recusal here is the
president on tape and also trey Gowdy weighing in on this he should have told
me before he took the job and I would pick somebody else if I were the
president and I picked someone to be the country's chief law enforcement officer
and they told me later oh by the way I'm not going to be able to participate in
the most important case in the office I would be frustrated to learn I think the
the sound you played from the president was from July of last year he's still
mad about it I mean he's always going to be to be mad about it because he
believes that the grounds that sessions used to recuse himself were knowable
before the sessions was even appointed attorney general and I think when trey
Gowdy said hey maybe the the sessions could have said wait a minute I
have to recuse myself from this extremely important case president is
clearly still mad about it but frustrated because the chairman of the
Senate Judiciary Committee who would be responsible for confirming a successor
as Attorney General has basically said that the White House

I thought when I became a women's health nurse practitioner that I would love the
OB --with its prenatal care and anticipation-- and that I would hate the
Gyn (for the gynecology). The OB part ended up being so very repetitive, and the young
adolescents whom I served needed so much educational support that I ended up
actually enjoying the gynecology more. There was a little repetition. You never
knew what the problem would be when someone called for a gyn appointment. One
day a teenage girl called to make an appointment. "And what will the clinic
visit be for?" asked the medical assistant. "I... I have a stick in my back," replied the patient.
The medical assistant called me to the front office to take the call
because it seemed so unusual.
The patient was clearly uncomfortable talking about it.
She repeated that she had a stick in her back. I told her if she had any sort
of a spinal injury she should go immediately to the ER.
"No no!" she say, "It's not like that! You're the one I need to see." We managed to get her in the next day.
Her mother came with her, and that was a very good thing. I'm not sure I
would ever have found out what the real problem was if I had had to rely on her
description and unwillingness to allow me to see the problem area.
She had a large hemorrhoid.
Women are just as a reluctant or more so to discuss sexually
transmitted infections. Our society somehow considers it to be a moral
failing for women to be promiscuous, though a promiscuous man is often
considered viral or cosmopolitan. All pregnant women and all annual gyn exams
include a sexually transmitted infection panel, which is HIV, hepatitis B, RPR,
(which is for syphilis), cervical cultures for chlamydia and gonorrhea, a wet prep
for trichomonas, and yeast, and bacterial vaginosis. While we're at it though, those
usually (meaning yeast and BV) are not considered sexually transmitted infections. Visual inspection
of the vulva may reveal the presence of condyloma
from human papilloma virus, though it is unlikely
since most forms of human papillomavirus leave no visible trace of their
existence. If the patient is having a herpes outbreak, this can be visualised
but it is always heralded by acute pain and that is usually the presenting
complaint rather than simply an annual exam. Usually of course all of these
exams come back negative but when they do come back positive, the clinician must
report the reportable infections to the health department via an official form.
The client must be informed. Most clinics have policies to the effect that
sexually transmitted disease notification must be given face to face.
We would not want a patient with positive HIV status to commit suicide,
for instance, nor do we want to give a result over the phone to someone who
claims to be the patient but is not. So the patient returns to the clinic
because we have told them over the phone that we need to discuss the results with
them. They're usually a bit worried. I have had many girls that have just
become sexually active sitting in my office awaiting their results. Sometimes
the boyfriend or husband is with them, and I give them the choice to speak with
me alone before we even go into the office.
Usually, however, they want the man there for support. I give them the news and
usually it is chlamydia. I explain that is it is a sexually transmitted
infection. Some women react calmly and it has been the policy that the men and
women may both receive prescriptions for this-- which is actually the only thing
for which a women's health nurse practitioner can treat a man. One young
girl said. "Well it must be wrong because I've only had sex with one man and it's
him, so I can't have a sexually transmitted
disease!" she pointed to her boyfriend... and he was by this time looking away, staring
up at the ceiling. And I would tell her, " The test is quite conclusive-- it's very
reliable, and when I did the exam the appearance of the mucus in your cervix
did appear to be the sort that we see when chlamydia is present. "But it can't
be!!" she would say! And suddenly, as the truth of the
situation dawned upon her, she started crying and shouting and slapping her
boyfriend on the head and face. He looked sheepish and held her hands
firmly to prevent further hitting. I gave them instructions and prescriptions, and
they quickly left. Usually the notifications were not so dramatic. One
day we had a case of chlamydia in a newly pregnant woman. Her husband was
there, and this particular day the physician was also there and said he
would take care of talking with them. The woman came out of the room a few minutes
later with a tear-stained face, but the doctor remained in the office speaking
with the husband. I could hear raised voices and soon the two of them stormed
out, and the couple left. My point is that infections in any other context do not
often cause emotional, psychological, and marital pain. Human sexuality is not just
a branch of physiology. It is inextricably linked with our deepest
feelings, and our self concepts, and self-image. Many women in my office with
a sexually transmitted infection displayed the merest sign of impatience
that now they would need to shell out money for an antibiotic. But it
seemed not in any way to be linked with emotional pain for them. Every situation,
though, is different. Practicing as a woman's health nurse
practitioner freed me in many ways. I learned to simply take each individual
as they were, and deal with the situation at hand. I separated my own conventions
and morality from how I treated them. It is always right to advise patients to
safer practices, and that is an integral part of Nursing. But the patient herself
is the one who determines what she will do about it. I care for her no matter
what choices she makes. And if she comes back to the clinic in a month with the
same sexually and transmitted infection, we start all over and I continue to be
her clinician. I found out a long time ago that the world doesn't revolve
around what I think is right or wrong, and that I must care for each patient
the best way I can--- seeking their health, and moving from their
current health status to where they and I together want them to reach. Note
the box on page 175 (disregard page number) identifying the sexually transmitted infections. In
general, remember that we can treat and cure all of the STIs except the viral STIs
(or sexually transmitted infections). Resistant varieties of some of these
others are beginning to emerge so we may not be able to cure all of them for long,
unfortunately. You will need to know the manifestations of each one of the STIs, though
you do not have to memorize all of the treatments and dosages. I would have some
general idea of the treatment, however. For instance, that penicillin is always
the treatment for syphilis. Remember that when you treat a pregnant woman the
fetus is also being dosed. Some medications are not appropriate for
pregnant women, and so we have to use something different to treat the the
disease... something that may not be as efficacious but that is safe for the baby.
So what is the most common STI ? Without doubt that would be human papilloma
virus or HPV. An authority in women's health, Sherry Carter, from UT
Southwestern, told us in a class that if a woman has had sexual relations with
two people, or if her partner has, she HAS been exposed to HPV. That includes just
about everyone. HPV is extremely contagious! A healthy immune system can
eventually clear this virus from the system. It does seem, however, to get worse
during pregnancy due to immune system changes. And if there are visible warts,
they can exacerbate to the point that a vaginal delivery is not possible because
of the sheer amount of space they consume, though that is rare. There are at
least a hundred serotypes of this disease and only about five of them
cause visible warts. About eight of the serotypes of HPV can cause cervical
cancer, and in fact are the primary cause of it.
The most common of the bacterial STI is is chlamydia. I had thought when I went
into women's health that I would see all of the STI is on the list on page 175 (disregard page number). In
reality I saw lots of cases of chlamydia and a few cases of
gonorrhea, herpes, and trichomonas. Note that Group B Strep
is tested in pregnancy but it is common flora for many women and is usually not
a sexually transmitted infection. And it is very common. I never did get to see
syphilis in my own practice, except as a clinical student at the Dallas County
Health Department. Small-town USA really doesn't get many of the more exotic
diseases, and I practiced in a very small town on the Texas-Mexico border.
So which STI hurts the most? Herpes, every time! Every time I got a call from a girl
saying , "It just hurts so much!" it turned out to be herpes simplex. Type 2 is
mentioned as the genital sort of herpes but herpes is not particular. Type 1 is
quite content to grow on the genitalia. The worst case I ever saw with at least
20 lesions and which was causing a lot of pain to the young girl was type 1. In
our discussion of how it is transmitted, she denied sexual activity with anyone
other than her boyfriend who was with her. He did not and never had had genital
herpes. I could tell she was telling the truth when denying promiscuous sexual
activity. As I was explaining the use of acyclovir, I happened to glance up at the
boyfriend and noticed for the first time a large herpes cold sore on his lower
lip --which proved to be the source of the infection for the girl. It is important
to know that many of the STIs, particularly chlamydia and gonorrhea, may
have no symptoms at all at first. A few months can go by without any noticeable
change in vaginal secretions or pelvic pain. But sometimes, there are
manifestations relatively soon after infection, including increased pain with
menses or purulent cervical discharge. Of course either of these infections can
lead to generalized pelvic infection known as pelvic inflammatory disease or
PID. Every case truly is different. Most of the women I diagnosed had come in for
pregnancy or annual exams and not because they had suspicions that they
had an STI. The TORCH infections frequently show up
on standardized tests, so make sure you know them. You have already studied
the sexually transmitted diseases and prevention for those in Health
Promotions, so we won't duplicate that.
What you need to learn from this chapter
is the STI s and other infections like TORCH and how they relate to pregnancy
The table on page 179 (disregard page number) in the book is quite helpful. Most women have BV (bacterial vaginosis) at some
time since it is not necessarily sexually transmitted, and it may be just
an overgrowth of the normal indigenous bacteria. But we believe it can cause
preterm labor and birth page 170 excuse me 194 (disregard page numbers). Perhaps the bacteria involved
actually weaken the membranes that surround the baby and cause preterm
rupture, or maybe toxins produced by the bacteria stimulate prostaglandin
production and initiate contractions. We don't know. Trichomonas vaginalis is a
one cell protozoa that is almost always sexually transmitted, though not every
single time. It is easy to see under a microscope-- it "bebops" and gyrates and
vibrates while most of the other cells that you're looking at under the
microscope are very still. It can live in a male or female body for a long time
without very many symptoms and we do treat it during pregnancy. Group B Strep
turns out to be a very big problem. We treat it in pregnancy.
Most of us test by doing a vaginal culture on admission, and if it's
positive we treat with the course of oral antibiotics. We retest again at 36
weeks, and treat again if it's present. Then, when the woman goes into labor we
give IV antibiotics while she's in labor. All of this to prevent the baby getting
it because it can cause neonatal sepsis or respiratory infections, and
maternal fever. All from a bacteria that is truly a part of the normal flora and
the non pregnant women, present in 9 to 23% of pregnant women and that would be
from page 197. (Disregard page numbers). What if a woman is attended by a midwife and is Group B
Strep positive? What happens then? They test for this!
Professional Midwives will send a woman to a physician for a prescription
Certified Nurse Midwives, of course, are advanced practice nurses and they can
prescribe the necessary medications in inpatient and outpatient settings.
Whenever I go down to donate blood I notice that they mark my blood CMV
positive. Cytomegalovirus: sixty percent of the
adult population in the US is CMV positive. When I was a Pedi ICU nurse, early
in my career, I cared for many cases of neonatal cytomegalovirus, some of which
were very sad. I imagine some time during that period I
contracted CMV. We want to prevent women from getting their primary infection with CMV
during pregnancy because it can affect the fetus profoundly, though the mother
herself may show no symptoms at all.
